The recent study by Calhoun and Hokanson1 was well designed and carried out. A careful reading of the paper provides practicing primary care physicians with helpful information in choosing antibiotics for their patients with acute sinusitis.
I disagree, however, with the wording of the conclusion, which states, "In conclusion, clarithromycin (500 mg twice daily) is an appropriate agent for the empiric treatment of acute maxillary sinusitis." The title and conduct of the study implied a comparison of two drugs; to conclude in the final sentence of the article (as well as in the abstract) about only one of the drugs is misleading.
A more consistent and correct conclusion would be "Clarithromycin and amoxicillin are equally effective agents for the empiric treatment of acute maxillary sinusitis." To which I might add, "Because amoxicillin is far less expensive than clarithromycin, it should be the agent of choice in the empiric treatment
