THE ARTICLE on the possible association of urinary tract infection (UTI) with a recent pelvic examination in this issue of the ARCHIVES1 demonstrates an association of the presumptive diagnosis of a U T I in women who have had a recent pelvic examination compared with those who were being seen for sinusitis. If a causal relationship is eventually shown, research on interventions to reduce this risk would clearly be indicated. As stated by the authors, the methods used in this study were not sufficient to explore causality, but they do suggest a need to clarify this issue.
Several risk factors for UTIs have been reported previously such as young age, sexually active status, urinary manipulation, diaphragm use, and history of previous UTIs. It is well recognized that in many women, the cause of a current UTI is unclear, and other as yet unrecognized factors may be playing a role.
