Approach to the patient with known or suspected coronary artery disease undergoing elective surgery. For all patients undergoing surgery, antianginal medications should be taken with a sip of water on the morning of surgery; aspirin therapy should be discontinued for 7 to 10 days; and diuretic therapy should be discontinued on the morning of surgery. Asterisk indicates preoperative risk indexes described by Goldman et al23 and modified by Detsky et al24 PTCA, percutaneous transluminal coronary angioplasty; and CABG, coronary artery bypass grafting.
